Single Point of Entry for Long Term Care in New York

In the Fall of 2007, NY State rolled out "phase 1" of its single point of entry for long term care (POE) program, entitled NY Connects. This page will be updated as the program is rolled out and more information is available.

NY Connects is charged with providing New Yorkers with access to information on long term care.  This includes information on: community based services, such as home care services, hospice services and services for specific populations (like the elderly, people with traumatic brain injury and children with special needs); services in health care facilities, such as hospitals, nursing homes and veterans homes; housing services; transportation services and more. NY Connects also provides information on various funding options.

NY Connects is set up to function on a county wide basis, not a state wide basis.  For consumers and the public, this means that they must access POE services from the contractor in their county, not from the state directly.  Each county has a different contractor and may be providing POE services in a different way.  The exception is New York City, in which all five counties that make up the city will be served by the same POE program. May, 2008. New York City, at this time, does not have a contractor to implement NY Connects. In a letter dated March 4, 2008 the city, in withdrawing from the state contract, stated that the city would be focusing on the “City for All
Ages” initiative that “will encompass all efforts now underway in New York City to assess age friendliness and livability of our city, and to marshal public and private resources in addressing the needs of our aging and disabled population.”

We will need to wait for another POE provider to be awarded the contract.

Although LTCCC has released a report on the first year of the POE, (click here for a copy of the report) LTCCC will continue collecting surveys of New Yorkers' experience with NY Connects, the single point of entry for long term care.  This survey will continue to help us identify how well the POE is serving New Yorkers.
